Three of the bulbs are 100 watt GE Reveal bulbs (they're supposed to cancel out the yellowness of regular incandescents). The other bulb is the same brand, but only a 60 watt. It's the second one from the left, and I use it to soften some of the shadows a bit.
The paper is Bristol Board, but I've used plain old posterboard too. Also, there's interfacing around the box to diffuse the light. I can't claim credit for coming up with the idea though, it's very heavily influenced by Lindsay's setup: http://theswitchboards.com/articles_professionalphotos.html.

I found lamps on Ikea's website, but it says the max is 60W.
